Vaccination Information

Ohio University requires that all residential students disclose their current meningitis, hepatitis B, and COVID-19 vaccination status prior to residing on-campus.

Meningitis and Hepatitis B

The Ohio Revised Code (ORC) Section 1713.55 states that beginning with the academic year that commences after July 1, 2005, an institution of higher education shall not permit a student to reside in on-campus housing unless the student (or the student’s parent if the student is younger than 18 years of age) discloses whether the student has been vaccinated against meningococcal disease and hepatitis B by submitting a meningitis and hepatitis B vaccination status statement.

Please note that the state of Ohio law does not require vaccination of the student, nor does it require the institution to provide or pay for vaccines. The law requires only the disclosure of vaccination status of the student.

COVID-19

Ohio University requires that students who reside in on-campus housing disclose whether they have been vaccinated against COVID-19 prior to residing on-campus.

Students are not required to have been vaccinated, nor receive the booster at this time for COVID-19. Students are required to indicate if they have been vaccinated, and if they have, when and what types of vaccines were received. 

Vaccination Information Form  

By completing the Vaccination Information Form, the student will disclose their vaccination status and acknowledge that they have read and understood the risks associated with meningitis (view meningitis PDF) and hepatitis B (view hepatitis B PDF), and the information listed on the Ohio Department of Health’s website as required by ORC Section 3701.133.  

In addition, the student acknowledges that they have read and understood the risks associated with COVID-19, along with and other information from the Centers for Disease Control and Prevention website and that COVID-19 infection or exposure may prevent students from attending class and other campus activities.
